Short answer
What this tool is: a converter that turns a milligram figure into a syringe unit reading, using nothing but the vial size, the water volume, and the barrel scale typed in above.
Why milligrams alone settle nothing: a syringe unit measures volume and a vial label states mass, so no unit mark carries a milligram value until the concentration exists. Concentration is vial milligrams divided by water milliliters.
What the approved label pins down: the Mounjaro prescribing information lists six single-dose presentations from 2.5mg to 15mg per 0.5mL, and prints six more concentrations for the multi-dose vials and KwikPens. There is no one brand concentration.
What the trials measured: SURMOUNT-1 assigned 2539 adults to 5mg, 10mg or 15mg once weekly or placebo for 72 weeks and reported mean weight change of -15.0%, -19.5% and -20.9% against -3.1% on placebo.

What three numbers does the arithmetic need?
The vial states mass: the milligram figure printed on a vial is a claim about how much compound is inside it, and that figure appears nowhere on any syringe barrel.
The barrel states volume: a U-100 mark is 0.01mL of liquid and a U-40 mark is 0.025mL, whatever happens to be dissolved in it.
Water joins the two: milligrams divided by milliliters gives concentration in mg per mL, and concentration divided by the barrel's units per mL gives the milligrams sitting inside one mark. Every figure the converter prints is that pair of divisions, and nothing else on this page comes out of the tool.
Change any input and the whole answer moves: the same vial reconstituted with twice the water halves the concentration and doubles the unit count for an identical milligram amount. That is one of the three assumptions two charts can differ on, and it is the one with no fixed ceiling, since a scale change can only move a count by 2.5x while a water volume can be anything.
What the Mounjaro label specifies
The presentations, as listed: section 3 of the current prescribing information gives single-dose pens and vials at 2.5, 5, 7.5, 10, 12.5 and 15mg, each in 0.5mL, and separately gives multi-dose vials and single-patient-use pens that each hold four doses in 2.4mL.
Only one of those tables prints a concentration: the label states a strength per mL for the multi-dose vials and the KwikPens and leaves it out for the single-dose ones, so the concentration column in the first table below is division and the one in the second is label text.

Read the unit columns again: every single-dose presentation delivers the same 50 units of liquid, and every multi-dose dose the same 60 units. On the approved product the volume holds still and the strength is what moves. In the converter above the opposite is arranged: the water fixes one concentration for the whole vial, so a change in the milligram figure has nowhere to go except into the volume.
One worked example, start to finish
The setup: a 30mg vial and 3mL of bacteriostatic water, which puts the liquid at 10mg per mL. On a U-100 barrel each mark then holds 0.1mg, or 100mcg.

The honesty note: none of those milligram figures is a suggestion. They are round numbers chosen to make the division legible, and the converter above will run any other figure just as happily.

What the trials tested
The amounts the phase 3 program assigned
SURMOUNT-1, obesity: 2539 adults with a body-mass index of 30 or more, or 27 or more with a weight-related complication and no diabetes, randomized to once-weekly subcutaneous tirzepatide at 5, 10 or 15mg or placebo for 72 weeks, including a 20-week escalation period.
What it reported: mean weight change at week 72 of -15.0% at 5mg, -19.5% at 10mg and -20.9% at 15mg, against -3.1% on placebo, with adverse events causing discontinuation in 4.3%, 7.1% and 6.2% of the tirzepatide groups and 2.6% on placebo.
SURPASS-2, head to head: 1879 patients with type 2 diabetes over 40 weeks, tirzepatide at 5, 10 or 15mg against semaglutide at 1mg, with mean baseline glycated hemoglobin of 8.28%.
What it reported: glycated hemoglobin change of -2.01, -2.24 and -2.30 percentage points against -1.86 for semaglutide, and greater weight reduction with tirzepatide at every dose, the least-squares mean treatment differences being -1.9kg, -3.6kg and -5.5kg.
Why the amounts sit here and not in the converter: those are the doses two named trials assigned to enrolled participants under supervision. They are a record of what was studied, and the tool above has no opinion about any of them.
How many units is 2.5mg of tirzepatide?
There is no single answer, because the unit count depends entirely on how much water went into the vial. At 10mg per mL, which is a 30mg vial with 3mL of water, 2.5mg occupies 0.25mL and reads as 25 units on a U-100 barrel. The same vial made up with half that water sits at 20mg per mL, so the identical 2.5mg reads as 12.5 units. A chart that answers this question with a single number has settled on one concentration to get there, and the number is only usable if that concentration matches the vial in front of you.
What concentration is a tirzepatide pen?
On the approved product there is no one concentration to quote. The label puts every single-dose strength into 0.5mL, so a 2.5mg presentation is 5mg per mL while a 15mg presentation is 30mg per mL, a sixfold spread across the same product line. The multi-dose vials and the KwikPens each hold four doses in 2.4mL, and the label prints their concentrations as 4.17 to 25mg per mL. A reconstituted vial has no such fixed relationship at all, which is why the converter asks for water before it will answer anything.
Why do two tirzepatide unit charts disagree?
Three hidden assumptions: vial size, water volume, and barrel scale. A chart that prints a unit count has fixed all three, whether or not it says so.
Water volume is the one with no ceiling: a 30mg vial reconstituted with 3mL sits at 10mg per mL, and the same vial with 1.5mL sits at 20mg per mL, so every unit count in the second chart is half the first. Any ratio of water volumes produces that ratio in the counts.
The barrel is the fixed one: a count derived on a U-100 barrel and read on a U-40 pulls 2.5x the liquid, always by that factor and always in the same direction, because the two scales divide the same milliliter into 100 marks and into 40.
Does the syringe scale change the milligrams?
No. The scale changes only how much liquid sits behind a numbered mark, and the milligrams inside that liquid come from the vial and the water alone. Thirty units on a U-40 barrel is 0.75mL where thirty units on a U-100 is 0.3mL, so at one fixed concentration the U-40 mark carries two and a half times the compound. That is why the converter treats the scale as a required input rather than a detail, and why it refuses to print a per-unit value until one is chosen.

Two kinds of number appear above: figures taken from a named trial abstract or from the current prescribing information, and figures this page derives by division. Where a figure is label text it is marked as printed, so the two are never mixed in one column.
Every derived figure is re-runnable: the concentration column in the first table and the unit columns in both come from the same two divisions the converter performs, so any of them can be checked with a calculator in about ten seconds. The concentration column in the second table is not derived here; it is the label's own figure at the label's own rounding.
The diluent named in the worked example has its own label: bacteriostatic water is a specific product with its own prescribing information, cited below, and the worked example uses it as the water in the arithmetic rather than as a claim about the compound.
