Concentration vs dose: the mix-up behind most dosing mistakes
Short answer
The definition: concentration is milligrams per milliliter, a property of the liquid sitting in the vial. A dose is milligrams, a decision measured against that property.
Why the mix-up costs: read a concentration as a dose, or a dose as a concentration, and the drawn amount lands at double or half of what was intended.

The two definitions
Same vial, two numbers: every mixed peptide vial carries both figures at once, and they answer different questions. Keeping them apart is the entire game.
Concentration: a property of the liquid
What it states: how much peptide is dissolved in each milliliter, written as mg per mL.
Who sets it: the vial size and the water volume, fixed the moment the two are mixed. Nobody chooses it at the syringe.
Where it shows up: in reconstitution notes and mixing logs, for example 5mg per mL.
Dose: a decision in milligrams
What it states: how much peptide a single administration is meant to contain, written as mg.
Who sets it: whoever planned the protocol, working inside whatever concentration the liquid already has.
Where it shows up: in trial arms and protocol write-ups, always as a plain milligram figure with no mL attached.
The one-line test: ask which question the number answers. "How strong is the liquid?" is concentration. "How much peptide is in this one injection?" is the dose.
How the mix-up doubles or halves a result
The first swap: treating the vial's total milligrams as the dose. A vial labeled 5mg holds 5mg spread across all of its liquid, not inside any one draw.
The second swap: treating mg per mL as a dose. 5mg per mL describes every milliliter in the vial; what lands in a single injection depends on how much liquid gets pulled.
The third swap: borrowing a unit count from a chart built at a different concentration. A chart that marks 20 units for 1mg at 5mg per mL delivers 2mg at 10mg per mL, exactly double, because each unit there carries twice the peptide.
And the mirror: the same 20-unit mark at 2.5mg per mL holds 0.5mg, exactly half. Same barrel line, same steady hands, a different result in both directions.
Why it keeps happening: the syringe reports volume and nothing else, so every one of these errors looks identical on the barrel. The mistake stays invisible until the milligrams get re-derived.
One worked example, two concentrations
The setup: a 5mg vial, bacteriostatic water, and a U-100 syringe. The only variable is the water.
Mix one: with 2mL of water in the vial, the liquid sits at 2.5mg per mL. A 1mg amount occupies 0.4mL, which the barrel reports as 40 units.
Mix two: with 1mL instead, the concentration doubles to 5mg per mL. The same 1mg decision now occupies 0.2mL, or 20 units.
What changed: the dose never moved. Only the liquid did, and the syringe reading followed it.
What would break: carry the 40-unit habit from mix one into mix two and the injection holds 2mg, double the plan. That is the whole cautionary tale in one line.

How to read it: every row below is the identical milligram amount; what moves is the volume that has to carry it. The table is arithmetic on the stated concentrations, not a chart for any vial in anyone's hands.

What is the difference between concentration and dose?
Concentration is how much peptide each milliliter of a liquid holds, written in mg per mL, while a dose is how much peptide one administration is meant to deliver, written in mg. The first is a property the liquid gets at mixing time; the second is a plan, measured out against the first. Neither figure can be checked without the other, which is exactly where the confusion breeds.
Is 10mg per mL stronger than 5mg per mL?
Per milliliter, yes: each drop of the 10mg per mL liquid carries twice the peptide, but that says nothing about the dose, because the dose is the milligrams actually measured out. At 10mg per mL a 1mg amount fits in 0.1mL; at 5mg per mL the identical 1mg needs 0.2mL. Stronger liquid, smaller volume, same milligrams. "Stronger" describes the liquid, never the plan.
How do I go from concentration to units?
Divide the milligram amount by the concentration to get the volume in milliliters, then multiply by the syringe's units per milliliter. At 2.5mg per mL, a 0.5mg amount works out to 0.2mL, which a U-100 barrel reports as 20 units and a U-40 barrel as 8 units. The converter linked below runs that division in both directions.
Does adding more water change the dose?
No; extra water lowers the concentration and leaves the vial's total milligrams untouched, so the dose only changes if the measured amount changes. A 5mg vial holds 5mg with 1mL in it or 3mL in it. What shifts is the landscape: at 3mL the liquid reads about 1.67mg per mL, and a 1mg amount spreads across 0.6mL, or 60 units on U-100.
