Needle gauges and syringe sizes: what the labels specify
Short answer
The definition: gauge is a size designation for needle tubing, standardized in ISO 9626, and it runs backwards. The number climbs as the tubing gets narrower, across a range the standard covers from 10 to 34.
What the pen labels specify: the Ozempic multi-dose pens and the Wegovy FlexTouch pen ship 32G pen needles 4mm long, with compatible needles up to 8mm named. The Genotropin pen instructions list 29G, 30G or 31G.
What a vial kit specifies: Egrifta SV supplies two different needles for one dose, a 1 inch 20-gauge for mixing and a half inch 30-gauge for injecting.
What a depot specifies: Sandostatin LAR Depot supplies a 1.5 inch 19 gauge safety needle for its gluteal injection.
The scope: every figure on this page is either a label reporting a specification for its own product or a clause of ISO 9626. Where the Egrifta SV label's own mixing steps are described, they are reported as that label's text, and nothing here is a recommendation for anything else.

What a gauge number designates
The standard: ISO 9626, second edition, published 2016, covers stainless steel needle tubing for medical devices and sets out size designation and dimensions in its own clauses. Tubing size is designated by nominal outer diameter, the corresponding gauge, and wall thickness.
The inversion: a bigger number means a thinner tube, and the standard states the relationship without explaining where it came from, so we do not offer an origin story for it either. The standard's range runs from 10 at the wide end to 34 at the narrow end.
Why that trips people: the figure rises as the tube narrows, so it runs opposite to the quantity it describes. Two labels can print gauge numbers that look close while the tubing diameters are not.
What gauge does not say: length. Gauge is a diameter designation, so a label that cares about depth has to state a length beside it, which is exactly what the depot and vial kits below do.
The gauges approved labels actually name
How to read the table: each row is a specification one label gives for its own product, taken from the instructions for use or the how-supplied section. Nothing crosses between rows.

The span in one line: the widest needle on this table is the intramuscular depot needle and the narrowest is a pen needle for a clear solution. No label in this set gives a reason for the diameter it names, so the gap is reported here and not explained; the route page sets out what these same documents do say about route and preparation.
Where a label states a reason: Sandostatin LAR Depot names the 2 inch option for patients with greater skin to muscle depth. That is the only depth rationale printed in this set.
One dose, two kinds of needle, and the label says which is which
The kit contents: a 30-day Egrifta SV box is listed as 60 sterile 1mL syringes, 60 sterile 1 inch 20-gauge needles used for mixing, and 30 sterile half inch 30-gauge needles used for injection. Three needles are consumed per dose, only one of which touches a person.
Why there are two wide ones: the label puts one 20-gauge needle on a fresh syringe to move sterile water into the vial, then a second 20-gauge needle on a second fresh syringe to withdraw the mixed solution, before the 30-gauge injection needle goes on.
What that separation encodes: a rubber stopper and skin are different problems. The label prices them differently in both diameter and length, and it never asks the same needle to do both jobs.
The reconstitution note in the same section: the label describes the water running down the inside wall of the vial rather than onto the powder, because of foaming. Our reconstitution volume page covers what the water choice does to the arithmetic afterwards.
What the labels say about syringe size
The tirzepatide vial line: the Mounjaro label states that a syringe appropriate for dose administration is used, giving a 1mL syringe capable of measuring a 0.5mL or 0.6mL dose as its example, with a new syringe and needle for each injection.
Why those two volumes: the same label reports each single-dose pen or vial as 0.5mL of solution, and each multi-dose presentation as 2.4mL delivering four doses of 0.6mL. The syringe spec is downstream of the fill volume.
The insulin scale, which is a different animal: Humalog is supplied as U-100 and U-200, and its label states that no dose conversion is performed with the prefilled pens because the dose window already shows insulin units.
Where the scale is easy to misread: an insulin unit is a unit of biological activity, not a fixed volume, so the volume holding one unit depends on the concentration. U-100 puts 100 units in a millilitre and U-40 puts 40 in the same millilitre, so one unit of the second occupies 2.5x the volume of one unit of the first, which is 100 divided by 40 and nothing more. What does the gauge number on a needle mean?
It designates the outer diameter of the needle tubing, under a scale where the number rises as the tube gets thinner. ISO 9626 sets the size designation and dimensions for stainless steel needle tubing and covers gauges from 10 down to 34.
The practical read: a 32G pen needle is far finer than a 19 gauge depot needle, even though 32 is the larger number. Nothing about the printed figure is intuitive.
What it excludes: length, wall thickness and bevel. Labels that care about those state them separately, which is why Egrifta SV writes both a gauge and an inch measurement for each of its two needles.
Which needle gauges do approved labels specify?
Across the documents on this page, from 19 to 32. The intramuscular depot sits at the wide end, the vial-and-syringe products sit in the middle at 20 for mixing and 30 for injecting, and the pens sit at the narrow end at 29 through 32.
The pen figures, precisely: the Ozempic multi-dose pens and the Wegovy FlexTouch pen ship a 32G 4mm needle and name compatible needles up to 8mm. Neither label carries those figures for its single-dose presentations. Genotropin lists a 29G, 30G or 31G pen needle. Norditropin states compatibility with disposable needles up to 8mm long.
What no label in this set does: publish a gauge recommendation for a product other than its own, or for a compound that has no approved label at all.
Why do some labels specify needle length as well as gauge?
Because the target tissue sits at a depth, and gauge says nothing about depth. The pen labels that state a length work in millimeters: the Ozempic multi-dose pens and the Wegovy FlexTouch pen at 4mm supplied and up to 8mm permitted, and Norditropin naming compatibility up to 8mm without a supplied length. Genotropin names gauges and no length at all. The gluteal depot works in inches, at 1.5 inches supplied.
The one stated rationale: Sandostatin LAR Depot names a 2 inch 19 gauge needle, not supplied in the kit, for patients with greater skin to muscle depth. That is the only depth rationale any label in this set prints.
The non-injecting length: the Egrifta SV mixing needle is a full inch, which is about reaching through a stopper into a vial rather than about a person. Its injection needle is half that.
What syringe size do the labels specify?
Where a product ships in a vial, the answer is small and stated: Egrifta SV supplies 1mL syringes, and the Mounjaro label gives a 1mL syringe capable of measuring 0.5mL or 0.6mL as its worked example. Both documents state that a new syringe and needle are used each time.
Where the product is a prefilled pen: no separate user-filled syringe is involved. The dose window on a Humalog pen shows insulin units directly and its label states that no conversion is performed.

Why does one kit ship a mixing needle and an injection needle?
Because the two jobs need different tubing. The Egrifta SV box is listed with 60 one-inch 20-gauge needles for mixing and 30 half-inch 30-gauge needles for injection, so the wide one moves sterile water and the fine one goes into a person.
What the label does with the mixing needles: uses two, and discards both. The first sits on a fresh 1mL syringe and moves sterile water into the vial. The second sits on a second fresh syringe and withdraws the mixed solution. The 30-gauge injection needle goes on only after that.
The counting detail: two mixing needles per dose against one injection needle, which is why the 30-day box holds 60 of one and 30 of the other.
