Bacteriostatic water vs saline: which one the labels specify

Short answer

The difference in one line: saline is salt water its own label calls isotonic, and bacteriostatic water is water with a preservative and no salt, so the two differ on tonicity first and on preservation second.

The numbers behind that: 0.9% sodium chloride injection carries 154mEq per liter each of sodium and chloride and an osmolarity of 308 mOsmol per liter. Bacteriostatic water carries no sodium chloride at all.

There are four products, not two: water and saline each exist in a preserved and an unpreserved version, and both preserved ones carry benzyl alcohol, at 9mg per mL in the saline and at 9mg or 11mg per mL in the water.

What only saline can do: the water labels state that the preparation must be made approximately isotonic before use, and warn that intravenous administration without a solute may cause hemolysis. Saline arrives isotonic.

What the preserved saline label rules out: flushing intravascular catheters, for which it directs preservative-free sodium chloride injection instead.

The four products, and what separates them

Two variables, not one: whether there is salt in the liquid, and whether there is a preservative in it. Every product in this family is one combination of those two answers.

The coincidence worth noticing: in the preserved saline, the sodium chloride and the benzyl alcohol are both present at 9mg per mL. The label states them as two separate line items, the salt as the solute and the benzyl alcohol as an added preservative, and the numbers being equal is arithmetic coincidence rather than a relationship between them.

Tonicity is the line that divides them

What saline brings: its label states 900mg of sodium chloride per 100mL, electrolytes of 154mEq per liter of sodium and 154mEq per liter of chloride, and a calculated osmolarity of 308 mOsmol per liter, which it describes as isotonic.

What water does not: both water labels carry the requirement that the preparation must be made approximately isotonic prior to use, and the sterile water carton prints not isotonic and hemolytic in capitals.

The shared warning: intravenous administration of either water product without a solute may result in hemolysis, and neither should be given intravenously unless the osmolar concentration of the additives brings the mixture close to isotonic.

Why it matters for a diluent: the water labels set the condition on the finished admixture rather than on the water, requiring that the osmolar concentration of the additives result in an approximate isotonic admixture, and they send the choice of vehicle and dilution to the manufacturer of the drug being dissolved rather than answering it themselves.

What does the preserved saline label rule out?

Not for flushing lines: the warnings section states that preservative-free sodium chloride injection should be used for flushing intravascular catheters.

Not for replacement: the contraindications add that parenteral preparations with benzyl alcohol should not be used for fluid or sodium chloride replacement.

Not for the spine: the same section states that parenteral preparations containing benzyl alcohol should not be used in epidural or spinal anesthetic procedures.

Not in newborns: due to the potential toxicity of benzyl alcohol in neonates, solutions containing it must not be used in that population, and the label names preservative-free sodium chloride injection as the alternative for preparing or diluting their medications.

The pattern across all four: every restriction above is written about the preservative rather than about the salt, and the unpreserved saline label separately lists no known contraindications at all.

Where the two preserved labels read identically

The indication sentence: both state that the preparation is indicated only for diluting or dissolving drugs for intravenous, intramuscular or subcutaneous injection, according to instructions of the manufacturer of the drug to be administered.

The handling instruction: both direct aseptic technique for single or multiple entry and withdrawal from all containers, and both say to mix thoroughly and use promptly.

The storage instruction: both say not to store reconstituted solutions of drugs for injection unless the manufacturer of the solute directs otherwise. It is the only storage instruction either label gives for the mixture, and it points away from the diluent entirely.

The inspection instruction: both require the reconstituted drug to be inspected for clarity and for freedom from unexpected precipitation or discoloration before administration.

Can saline be used instead of bacteriostatic water?

Both labels name the same indication, and both push the actual choice to the same place: consult the manufacturer's instructions for choice of vehicle. That sentence appears in the precautions of the water label and the saline label alike, and it is the closest either document comes to answering the question. What the labels do settle is the mechanical part, that plain saline is single-dose and unpreserved while bacteriostatic saline and bacteriostatic water are multiple-dose and preserved. The compound being dissolved decides the rest, and none of the four labels read for this page claims otherwise.

What is bacteriostatic saline?

Its own description: a sterile, nonpyrogenic, isotonic solution of sodium chloride in water for injection, each milliliter containing 9mg of sodium chloride and 9mg of benzyl alcohol added as a bacteriostatic preservative, and it may contain hydrochloric acid for pH adjustment.

Its container: multiple-dose, supplied in 10, 20 and 30mL vials, from which repeated withdrawals may be made.

Its pH: 5.0, with a stated range of 4.5 to 7.0, which is lower than the 5.7 stated for bacteriostatic water.

What makes saline isotonic and water not?

Dissolved particles. The saline label states a calculated osmolarity of 308 mOsmol per liter and calls the solution isotonic, and it gets there because the salt dissociates into sodium and chloride, 154mEq per liter of each. Neither water label states an osmolarity, and both instead state the opposite of the saline position: the preparation must be made approximately isotonic prior to use, and intravenous administration without a solute may result in hemolysis. Benzyl alcohol does not close that gap. It is listed as a preservative, at 9mg per mL in the preserved saline and at 9mg or 11mg per mL in the preserved water, and no label claims it contributes tonicity, which is why the bacteriostatic water label still requires the finished preparation to be made approximately isotonic.

Which one do the labels name for flushing a catheter?

The unpreserved one. The bacteriostatic saline label states plainly that preservative-free sodium chloride injection should be used for flushing intravascular catheters, and the bacteriostatic water label separately states that parenteral preparations with benzyl alcohol should not be used for fluid replacement. Both restrictions sit on the preserved products only, and both mark the same boundary: these labels indicate the product as a vehicle for diluting or dissolving a drug, not as a volume delivered for its own sake. It is one of the clearest places where the labels distinguish a diluent from a fluid.
